The shuttle carrying seven astronauts disintegrated over Texas just 16 minutes from landing. Everybody on board died. Months of investigation led to conclusion that a small hole on a panel of one of the wind caused hot gases to enter the shuttle during its re-entry to the Earth’s atmosphere.
